How are deliverables tracked against purchasing contracts?

I would like to understand how the whole process of tracking against a contract works in your organization. How are the deliverables added to the contract - is it in text in the clause or do you use terms or data definitions to track it? Where does that information go after the contracts are executed? Are the suppliers tracked to the SLAs in the contracts? Does this happen in a different application?
